Prepare for your visit to ApexNetwork

Here are seven simple, but important, things to remember for your next visit to ApexNetwork Physical Therapy®.

  1. A prescription ordering therapy is needed by your doctor.
  2. Workers’ compensation is accepted. Most insurance companies are accepted. But just in case yours isn’t, please check our listing.
  3. Our office manager will call for insurance approval.
  4. Please bring your insurance card to your first appointment.
  5. If you’d like, you can print, complete and bring a copy of our required health forms.
  6. Wear loose-fitting, comfortable clothing.
  7. Our office hours are 8:00am-5:00pm, Monday through Friday. We‘ll do out best to accommodate your schedule.
